We are seeking a skilled RN Care Manager to join our team. This is a rewarding opportunity to make a difference in the lives of our clients.

About the Role:
  • Conduct in-home nursing visits for clients with chronic/terminal illness due to exposure to toxic substances while working for the Government.
  • Provide direct care and case management of assigned clients, including physical assessments, home safety evaluations, medication planning, and oversight of caregivers.
  • Plan, implement, and evaluate care provided, participate in client care conferences as needed, and serve as a local on-site clinical resource.
  • Assist with marketing activities, such as visiting with clients or physicians to discuss Agency programs.
Requirements:
  • Graduate of a state-approved school of professional registered nursing.
  • Current, unrestricted RN license in the state(s) of practice.
  • Minimum of two (2) years nursing experience, including one (1) year in home care or closely related field.
  • One (1) year of supervisory and/or case management experience preferred.
Compensation:

$42.00 hourly plus shift differentials, where applicable. The estimated annual salary for this role is $87,840 based on 40 hours per week and 52 weeks per year.
