Job Overview

We are seeking a skilled Senior Case Manager to join our team at Professional Case Management. As a Senior Case Manager, you will be responsible for providing direct care and case management services to assigned clients.

Key Responsibilities
  • Provide direct care and case management services to assigned clients, ensuring adherence to Agency policy and state-specific nurse practice act guidelines.
  • Supervise and train RN Case Managers, promoting excellence in home care delivery.
  • Collaborate with the regional director and other personnel to identify and address issues, improving services and outcomes.
  • Plan, implement, and evaluate care provided, participating in client care conferences as needed.
Requirements
  • Graduate of a state-approved school of professional registered nursing.
  • BSN preferred; current, unrestricted RN license in the state(s) of practice.
  • Minimum two years of nursing experience, including one year in home care or a closely related field.
  • One year of supervisory and/or case management experience preferred.
Compensation Package

$44.00 per hour plus shift differentials, where applicable. This salary reflects the market rate for this role and location, ensuring fair compensation for your expertise and dedication.
