Job Summary

We are seeking a highly skilled and experienced Senior Case Manager RN to join our team. As a Senior Case Manager RN, you will be responsible for overseeing the provision of care for clients while maintaining compliance with agency policy and state regulations.

This role involves creating individualized care plans, managing a team of RN Case Managers, and ensuring quality care in home settings. You must hold an active RN license and have experience in nursing, with additional supervisory and case management experience preferred.

About the Role

The Senior Case Manager RN will be responsible for:

  • Providing direct care and case management of assigned clients
  • Assisting and collaborating with the regional director and other personnel to identify and correct issues and/or improve services
  • Planning, implementing, and evaluating care provided
  • Participating, coordinating, and managing client care conferences as needed
  • Serving as a local on-site clinical resource as needed and providing support to ensure client's home care needs are met
Requirements

To be successful in this role, you will need:

  • A graduate degree from a state-approved school of professional registered nursing
  • A current, unrestricted RN license in the state(s) of practice
  • A minimum of two (2) years of nursing experience, including one (1) year in home care or a closely related field
  • One (1) year of supervisory and/or case management experience preferred
  • Current CPR certification
