Job Summary:

We are offering a competitive salary of $100 Hourly plus shift differentials, where applicable. As a Nurse Care Coordinator Director, you will be responsible for overseeing the provision of care for clients while maintaining compliance with agency policy and state regulations.

Key Responsibilities:

  • Create individualized care plans to ensure quality care in home settings.
  • Manage a team of RN Case Managers to ensure effective coordination of care.
  • Ensure adherence to agency policies and state regulations.
  • Participate in performance improvement initiatives to enhance client care.

Requirements:

  • A valid active RN license.
  • Minimum of two (2) years nursing experience, including one (1) year in home care or a closely related field.
  • One (1) year of supervisory and/or case management experience preferred.
