At Professional Case Management, we are committed to delivering exceptional care to our clients in the comfort of their own homes. We are seeking a highly skilled and compassionate Senior Case Manager RN to join our team.

Overview

As a Senior Case Manager RN, you will be responsible for overseeing the provision of care for our clients while maintaining compliance with agency policy and state regulations. This role involves creating individualized care plans, managing a team of RN Case Managers, and ensuring quality care in home settings.

Key Responsibilities:
  • Create and implement individualized care plans that meet the unique needs of each client
  • Manage a team of RN Case Managers to ensure high-quality care is provided
  • Conduct supervisory visits to facilitate problem resolution and provide support to staff members
  • Review and evaluate clinical documentation for accuracy and completeness
  • Participate in performance improvement initiatives and collect data on infections, occurrences, complaints, and grievances
Requirements
  • Graduate of a state-approved school of professional registered nursing
  • BSN preferred
  • Current, unrestricted RN license in the state(s) of practice
  • Minimum of two (2) years nursing experience, including one (1) year in home care or closely related field
  • One (1) year of supervisory and/or case management experience preferred
  • Current CPR certification
Salary and Benefits

We offer a competitive salary of $44.00 per hour plus shift differentials, where applicable. Additionally, we provide a comprehensive benefits package, including bonus eligibility and opportunities for career growth and development.
