We are seeking a skilled Nursing Care Manager - Home Healthcare to join our team at Professional Case Management.

About the Role:

The Nursing Care Manager is responsible for overseeing the provision of care for clients while maintaining compliance with agency policy and state regulations.

This role involves creating individualized care plans, managing a team of RN Case Managers, and ensuring quality care in home settings.

Candidates must hold an active RN license and have experience in nursing, with additional supervisory and case management experience preferred.

Key Responsibilities:

  1. Direct assigned team members of RN Case Managers in the provision of care in accordance with Agency policy and with state-specific nurse practice act, and regulatory requirements
  2. Provide direct care and case management of assigned clients
  3. Assist and collaborate with the regional director and other personnel to identify and correct issues and/or improve services
  4. Plan, implement, and evaluate care provided Participate, coordinate and manage client care conferences as needed
  5. Serve as a local on-site clinical resource as needed and provides support to ensure client's home care needs are met
  6. Adhere to nursing delegation guidelines as described in Agency Scope of Practice policy
  7. Ensure adherence to Agency policies

Requirements:

  • Graduate of a state approved school of professional registered nursing
  • BSN preferred
  • Current, unrestricted RN license in the state(s) of practice
  • Minimum of two (2) years nursing experience including one (1) year in home care or closely related field
  • One (1) year of supervisory and/or case management experience preferred
  • Current CPR certification
