Job Title: Rapid Re-Housing Case Manager
Reports to: Housing Now Director
Job Summary: We are seeking a highly motivated and compassionate Rapid Re-Housing Case Manager to join our team at The Salvation Army. The successful candidate will provide housing support services to families in need, helping them achieve stability and independence.
Key Responsibilities:
- Develop and maintain relationships with program participants, landlords, and community resources
- Conduct assessments, identify financial interventions, and provide referrals to emergency assistance services
- Collaborate with the Housing Now team to ensure seamless service delivery
- Maintain accurate and detailed records, including case notes and WellSky data entry
- Participate in program events, such as Christmas distributions and food bank events
Requirements:
- Bachelor's degree in human services, social work, or a related field
- 2+ years of experience in housing services, preferably with homeless populations
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Bi-lingual skills a plus
