Job Title: Case Manager: Re-Housing and StabilizationJob Summary:

Volunteers of America Colorado is seeking a highly motivated and experienced Case Manager to join our team. The successful candidate will be responsible for providing case management services to individuals and families in need of housing and stabilization support. This is a full-time position that requires a strong understanding of housing and stabilization services, as well as excellent communication and interpersonal skills.

Key Responsibilities:
  • Conduct needs-based assessments to identify client needs and develop individualized service plans.
  • Provide case management services to clients, including housing navigation, support with income resources and public benefits, and landlord recruitment/mediation.
  • Collaborate with community service providers and multi-disciplinary teams to ensure comprehensive support for clients.
  • Accompany clients on appointments as needed and provide transportation for service provision.
  • Collect necessary documents to obtain and maintain housing, including identification documents, income changes, and changes in household.
  • Assist clients in their search for housing, including filling out rental applications, interpreting leases, and understanding tenant rights and responsibilities.
  • Ensure timely and accurate reporting within data entry systems and hard copy files in accordance with grant guidelines and data quality standards.
  • Create safety plans to support the safety and well-being of clients in the program.
  • Facilitate the equitable delivery of appropriate services for households as indicated by relevant service modalities, including Housing First, Trauma Informed Care, Harm Reduction, Motivational Interviewing, and Critical Time Intervention.
Requirements:
  • Bachelor's degree in human services, social work, or a closely related field.
  • Colorado driver's license and state-mandated automobile insurance.
  • Experience working with the literally homeless population and public housing authorities.
  • Strong written, oral, and interpersonal communication skills.
  • High level of competency working with Microsoft Office suite and cloud-based applications.
  • Strong time-management and prioritization skills.
  • Crisis and verbal de-escalation intervention skills.
  • Experience working both independently and in a team-oriented, collaborative environment.
Working Conditions and Physical Requirements:

This position requires the use of a vehicle for service provision throughout the seven-county Denver Metro area. The employee must be willing to work in close proximity to other staff residing at Bannock Youth & Family Center.
