Rapid Re-Housing Case Manager

Join our team at The Salvation Army as a Rapid Re-Housing Case Manager, where you will play a vital role in supporting families in need and helping them achieve permanent housing. As a key member of our team, you will work closely with participants, landlords, and community resources to provide comprehensive case management services.

Key Responsibilities:
  • Develop and maintain relationships with program participants, landlords, and community resources.
  • Provide case management services, including assessments, financial interventions, and referrals to community resources.
  • Collaborate with the Program Director and Case Managers to ensure effective program implementation.
  • Act as an advocate for participants, assisting them in stabilizing housing and accessing necessary resources.
  • Maintain accurate and detailed documentation, including case notes and WellSky data entry.
Requirements:
  • Bachelor's degree in human services, social work, or a related field.
  • Experience working with homeless populations, preferably with families in crisis.
  • Case management experience, preferably in a Rapid Rehousing program.
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • Ability to maintain confidentiality and exhibit loyalty to The Salvation Army and its staff.
