Rapid Re-Housing Case Manager

We are seeking a highly motivated and compassionate Rapid Re-Housing Case Manager to join our team at The Salvation Army. As a key member of our housing services team, you will play a vital role in supporting families who are experiencing homelessness and helping them achieve permanent housing.
Key Responsibilities:- Provide case management services to families in shelter, including housing search and navigation, assessments, and referrals to community resources.
- Develop and maintain relationships with landlords, property managers, and other stakeholders to secure housing opportunities for participants.
- Collaborate with other team members to ensure a comprehensive and trauma-informed approach to case management.
- Maintain accurate and timely documentation of case management activities and participant progress.
- Participate in regular team meetings and contribute to the development of program policies and procedures.
- Bachelor's degree in human services, social work, or a related field.
- At least two years of experience working with homeless populations, preferably with families in crisis.
- Strong communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
- Ability to maintain confidentiality and handle sensitive information with discretion.
- Valid driver's license and reliable transportation.
