Permanent Supportive Housing Case Manager

The Permanent Supportive Housing (PSH) Case Manager provides essential services to residents in a permanent supportive housing model, focusing on chronically homeless individuals. This role works in conjunction with the Community Administrator and other on-site staff to ensure residents receive trauma-informed care, housing assistance, and resource navigation. Key responsibilities include conducting needs-based assessments, community outreach, case management services, and assisting with resource navigation, housing retention, and landlord mediation.
Key Responsibilities- Conduct needs-based assessments and develop individualized service plans
- Provide case management services, including resource navigation and housing retention
- Assist with landlord mediation and ensure timely reporting within data entry systems
- Collaborate with service providers and multi-disciplinary teams to deliver services
- Accompany participants on appointments as needed
- Preferred Bachelor's degree in human services, social work, or a closely related field
- At least one year of experience working with chronically homeless populations
- Colorado driver's license and state-mandated automobile insurance
- Ability to obtain first aid, automated external defibrillator, and cardiopulmonary resuscitation certifications
