Housing Case Manager: Family Motel

The Housing Case Manager is responsible for providing service provision and housing assistance to families at the Family Motel. This program serves multiple populations, and the Case Manager will conduct needs-based and program eligibility assessments, engage in community outreach, and provide case management, crisis management, and de-escalation services.

- Engage in community outreach and provide case management services
- Provide crisis management and de-escalation services
- Connect participants to income resources and public benefits
- Determine rental amounts and recruit landlords
- Attend meetings and trainings, and collaborate with service providers
The Case Manager will ensure that the participant's voice is heard and their ideas play a prominent role in programming. They will possess an in-depth knowledge of program requirements, including the referral process, support services, allowable costs, field-relevant best practices, the sheltering system, and HUD permanent housing: rapid re-housing standards.
- Provide housing case management and ensure participants receive regular engagement
- Collect documents to obtain and maintain housing
- Identify and serve the unique needs of participant households
- Utilize screening and assessment tools and ensure the completion of needs-based screenings
The ideal candidate will have a Bachelor's degree in human services, social work, or a closely related field, and at least one year of experience working with people experiencing homelessness. They will possess good organizational and planning skills, work well as part of a team, and have a Colorado driver's license and state-mandated automobile insurance.
The Case Manager will also be responsible for developing knowledge related to service delivery database systems within the organization, attending intakes, housing orientations, lease-ups, and ongoing educational opportunities for participants, and ensuring timely reporting within data entry systems and hard copy files in accordance with grant guidelines.
CompetenciesThe Case Manager will model core culture attributes of VOACO, including Accountability, Integrity, Respect, and Service, and will utilize various databases, including Homeless Management Information System and Service Point. They will ensure equitable and inclusive services, and ensure that resident(s) have an active voice in programming.
