Position Summary

The Community Integrated Support Specialist plays a vital role in assisting individuals with mental health challenges by providing educational resources and connecting them with community-based support systems. This position involves active participation in the development and implementation of client treatment strategies.

Key Responsibilities:

  • Formulating client-focused treatment plans that include specific, measurable objectives.
  • Conducting meetings with clients as clinically warranted to assess their needs.
  • Tracking the progress of clients enrolled in waiver support services and other skill enhancement programs.
  • Ensuring compliance with DHS and Medica regulations, alongside maintaining client confidentiality standards.
  • Coordinating services and community resources effectively, collaborating with representatives from various agencies to enhance client support.
  • Maintaining accurate documentation and flow charts as required.
  • Completing all necessary paperwork promptly and attending mandatory meetings, training sessions, and supervision as needed.

Additional Information:

Driving is a requirement for this role, and work hours will be flexible based on client and program demands.

Compensation and Benefits:

  • Hourly Rate: $18.00

  • Standard 8-hour shifts, Monday through Friday.
  • Availability for weekend shifts may be necessary.
