Position Summary:

Become part of a transformative initiative aimed at reshaping the child welfare system by integrating substance use treatment within the home environment. This approach allows parents to focus on their recovery while keeping their children at home, rather than facing separation. The role involves providing individualized home-based support and community group activities that enhance parenting and recovery skills in alignment with each client’s case plan. Services offered include effective strategies for managing parental mental health, connecting families with community resources, and delivering parenting education and skill development. The specialist will maintain a caseload of 12 families, ensuring timely and effective support. This role is integral to the In-Home Recovery team, collaborating with both internal and external partners to meet the unique goals of each family. Accurate and timely documentation of client interactions is essential, requiring meticulous attention to detail and adherence to contract compliance and case noting standards. Participation in individual and departmental clinical supervision is expected. Key Responsibilities:

Case Management (60% - 70%):

  • Engage in the Yale Family Based Recovery Program training model, ensuring adherence with complete fidelity.
  • Provide home-based coaching and ongoing support that centers on family experiences through a respectful, trauma-informed approach, enhancing client satisfaction and retention.
  • Manage 12-14 active family cases, ensuring timely support and follow-up on all client needs as outlined in their case plans.
  • Facilitate community-based and internal referrals to families as required.
  • Ensure all client privacy is protected by completing necessary releases of information before engaging with external stakeholders.
  • Document all interactions and ancillary support within 24-48 hours, including communications with external stakeholders such as guardian ad litems and social workers.
  • Maintain an organized documentation system, including client files and related paperwork.

Group Management (10% - 20%):

  • Conduct weekly support groups for parents to explore and develop new parenting and stress management skills, promoting enhanced behavioral health.
  • Provide insights to group members that foster individual confidence and reflection.
  • Organize and coordinate weekly social clubs for families.
  • Document a summary of group activities post-social club, noting progress towards group objectives and individual member engagement.
  • Assist with managing vouchers for incentive-based components.

Operational Duties (10% - 20%):

  • Timely entry of case notes, assessments, and all intake forms for clients and groups, ensuring accurate and timely billing services.
  • Respond to internal information requests within 24-48 hours to facilitate colleagues' work.
  • Complete all required billing and reporting tasks by established deadlines.

Qualifications:

  • Bachelor's degree in family social services, social work, behavioral health/psychology, or a related field.
  • 2-3 years of experience working with families facing significant barriers to success and/or involvement with child welfare systems.
  • 2-3 years of experience assisting individuals and families in addressing behavioral health needs and objectives.
  • Fluency in Spanish, Somali, Hmong, and/or Karen is preferred but not mandatory.
  • Strong written communication skills, with the ability to share updates and inquiries with stakeholders, ensuring clarity and correctness.
  • Proficient calendar management skills to maintain an accurate record of work-related activities.
  • Experience collaborating within a team, contributing to collective goals, and addressing workplace conflicts professionally.
  • Strong oral communication skills, demonstrating clarity and persuasiveness in various situations, with an emphasis on active listening and effective responses.
