The Community Support Specialist (Social Worker) is responsible for conducting eligibility assessments for individuals with disabilities who receive home and community-based services or are at risk of institutional placement.

This role requires a certified MnCHOICES Reassessor who will assess eligibility for individuals with disabilities, developmental/intellectual disabilities, and older adults using the MnCHOICES tool.

The position may focus on two primary populations:

Under 65: Individuals under 65 with disabilities receiving support through various waivers.

Developmental/Intellectual Disabilities: Individuals with developmental/intellectual disabilities receiving support through the Developmental Disability waiver and related programs.

Minimum Qualifications

NEED TO HAVE:

• Bachelor's degree in Social Work, Psychology, Sociology, Human Services, Special Education, or a related field;
AND

• One year of full-time professional experience OR two years of full-time para-professional experience in a human service agency;
AND

• Required license(s): Valid Driver's license.

NICE TO HAVE:

• Master's degree and/or experience in relevant fields.

• Prior experience in home or community-based settings.

• Qualified Developmental Disabilities Professional (QDDP) status.

Duties & Responsibilities

Essential Functions:
  1. Engage individuals and families to assess needs and strengths, determine eligibility for services, and evaluate safety and risk levels.
  2. Conduct assessments, either virtually or in-person, for individuals with disabilities to determine eligibility for home and community-based services.
  3. Identify additional supports/services that may benefit clients, provide information and referrals, and collaborate with stakeholders.
  4. Develop and implement community support plans following relevant policies and professional practices.
  5. Complete required documentation for all assessments.
  6. Monitor client progress towards goals through regular reviews and service plan adjustments.
  7. Respond effectively in crisis situations and develop safety plans for clients.
  8. Participate in meetings and training relevant to the role.
  9. Support community emergency response functions as needed.
  10. Commit to promoting cultural diversity and supporting Dakota County's inclusion and equity initiatives.
  11. Other duties as assigned.
Knowledge, Skills & Abilities
  • Understanding of human development, cultural influences, and family dynamics.
  • Familiarity with complex service systems, including healthcare and community services.
  • Knowledge of medical terminology and community resources.
  • Ability to communicate effectively, both orally and in writing.
  • Ability to work independently and prioritize tasks.
  • Commitment to respecting diversity among clients and colleagues.
WORK ENVIRONMENT: Work is performed in various settings, including office environments and client homes, with required travel. Lifting of up to 20 pounds may be necessary, and exposure to challenging environments may occur.
