Job Summary

Radias Health is seeking a skilled Case Manager to join our Targeted Case Management team. As a Case Manager, you will play a critical role in providing person-centered services to promote recovery and community integration of adults with serious persistent mental illnesses and co-occurring substance use disorders.

Key Responsibilities
  • Develop and Implement Service Plans: Create functional assessments and community support plans to help clients achieve their goals and improve their quality of life.
  • Provide Mental Health Support: Engage with clients to discuss their mental health symptoms and service needs, and link them with needed psychiatric and medical providers.
  • Coordinate Community Resources: Refer clients to community resources, assist with financial and healthcare benefits, and communicate with providers and family members.
  • Monitor Civil Commitments: Monitor civil commitments, write required reports, and communicate with Ramsey County court programs.
Requirements
  • Education and Experience: Bachelor's degree in behavioral sciences or related field, and 2,000 hours supervised work experience with individuals diagnosed with serious mental illness and/or substance use disorders - OR - Master's or other graduate degree in behavioral health sciences or related field - OR - 4,000 hours supervised work experience with individuals diagnosed with serious mental illness and/or substance use disorders.
  • Skills and Qualifications: Strong communication and interpersonal skills, ability to work independently and as part of a team, and proficiency in electronic health records.
