Job Summary

The Case Manager provides comprehensive community-based case management services, including housing services and trauma-informed case management for individuals who are homeless and/or returning to the community from incarceration. This role serves as a liaison to various community resources and makes referrals as needed.

Key Responsibilities
  • Conduct interviews and assessments to identify the needs of returning citizens and individuals referred by jails or prisons.
  • Meet with community partners to identify areas for collaboration and develop plans for coordinating services.
  • Ensure participants engage in group sessions to address personal aspects of their lives, such as financial literacy and employment readiness.
  • Maintain accurate case records and report progress to the grant funder.
  • Foster collaborative relationships with organizations and service providers to support men transitioning to the community.
Requirements
  • Bachelor's degree in social work, human services, or a related field.
  • For social work degree, social work licensure in the appropriate jurisdiction is required.
  • 3 years of experience working with incarcerated adults, including those with mental disorders.
  • Ability to pass a background check and obtain access to secured facilities.
  • Valid local driver's license and clean driving record.
Skills and Competencies
  • Effective communication and interpersonal skills.
  • Ability to establish and maintain cooperative working relationships.
  • Proficiency in the use of computers and software applications.
