Job Title: Housing Case Manager

Collaborative Solutions for Communities is seeking a skilled Housing Case Manager to join our team. As a key member of our staff, you will be responsible for providing solution-focused, strength-based case management services to individuals and families in need of housing support.

Key Responsibilities:
  • Conduct home visits and community outreach to assess client needs and develop service plans
  • Connect clients with relevant referrals and community resources to achieve identified goals
  • Assist with outreach to and negotiation with landlords/property owners to secure safe and affordable housing
  • Develop and implement service plans in coordination with clients to address housing barriers and needs
  • Maintain accurate and timely case notes and program data in ClientTrack software
Requirements:
  • Bachelor's degree in social work, psychology, sociology, counseling, or related field
  • Minimum 2 years' experience providing case management services, preferably in homeless programs
  • Excellent oral, written, and interpersonal skills
  • Ability to pass a drug and TB testing, Metropolitan Police Department and Federal Bureau of Investigation background check, and Child Protective Services clearance
