Job Title: Housing Case Manager

Collaborative Solutions for Communities is seeking a skilled Housing Case Manager to join our team. As a key member of our organization, you will play a vital role in providing solution-focused strength-based family-centered case management services to individuals and families in need.

Key Responsibilities:
  • Conduct participant assessments to identify critical issues and develop personalized service plans.
  • Provide case management services, including connecting clients with relevant referrals and community resources, and assisting with outreach to landlords and property owners.
  • Develop and maintain relationships with community partners to ensure seamless service delivery.
  • Maintain accurate and up-to-date case notes and program data in ClientTrack software.
Requirements:
  • Bachelor's degree in social work, psychology, sociology, counseling, or a related field.
  • At least two years of experience in homeless programs or social services, with knowledge of community resources.
  • Excellent oral, written, and interpersonal skills.
  • Ability to pass a drug and TB testing, Metropolitan Police Department and Federal Bureau of Investigation background check, and Child Protective Services clearance.
Preferred Qualifications:
  • Covid vaccination required.
