Job Overview

St. Francis Center (SFC) is a vital organization dedicated to supporting individuals experiencing homelessness in the metro Denver area. Established in June 1983, SFC provides essential programs and services aimed at fostering self-sufficiency through daytime shelter, social services, health and wellness initiatives, housing assistance, employment support, and outreach efforts.

We are currently seeking a Housing Solutions Case Manager. This is a Full-Time, non-exempt role that comes with a comprehensive benefits package.

Position Summary

The Housing Solutions Case Manager will deliver shelter diversion and rapid exit services aimed at swiftly housing clients facing homelessness, thereby minimizing the necessity for emergency shelter. This role involves working closely with clients to evaluate their situations and identify effective solutions and stable housing options that promptly address their housing crises through a blend of direct services and limited financial support. The Case Manager will often engage with clients outside the conventional shelter environment.

Key Responsibilities
  • Collaborate with SFC intake and outreach teams, as well as other community service providers, to reach individuals in need of diversion services.
  • Assist clients in creatively identifying solutions and alternative housing options that effectively resolve their housing challenges.
  • Provide advocacy, mediation, and conflict resolution services with landlords, family members, or friends to secure or stabilize housing.
  • Act as a liaison among clients, support services, landlords, and community agencies, maintaining ongoing communication as necessary.
  • Evaluate clients' financial situations and their need for supportive services such as employment assistance, budgeting, and credit repair.
  • Facilitate connections to mainstream and community-based services and benefits.
  • Maintain accurate and confidential records of client interactions and financial assistance provided in required database systems.
  • Participate in regular staff and team meetings.
  • Support Direct Care services at the Day Center by working Day Shelter shifts, a minimum of 6 hours monthly.
  • Represent the agency in community or interagency activities and events as appropriate.
  • Engage with other shelters and service providers to promote and implement Rapid Resolution techniques.
  • Oversee Payee Safekeeping responsibilities.
  • Perform other duties as assigned.
Qualifications
  • Preferred: S.W. or equivalent credentials in a related field, or 3-5 years of experience in a case management role.
  • Essential: Two or more years of experience working with individuals in poverty or experiencing homelessness.
Knowledge, Skills, and Abilities
  • Ability to listen attentively, treat clients with respect and dignity, and respond to their unique needs.
  • Understanding of and ability to maintain professional boundaries.
  • Comprehensive knowledge of resources available in Denver and surrounding areas for individuals experiencing homelessness.
  • Basic computer skills, including proficiency in Microsoft Office, HMIS, and basic accounting software.
  • Strong problem-solving abilities, independent work skills, conflict mediation, crisis management, customer service, interpersonal, and oral communication skills.
  • Excellent interviewing skills with the ability to extract relevant resource information.
  • Capability to manage agency funds with discretion and accuracy.
  • Experience in data collection and record-keeping with a focus on detail and accuracy.
  • Basic understanding of Housing First principles, Trauma-Informed Practices, and motivational interviewing.
  • Willingness to work with individuals from diverse socioeconomic and cultural backgrounds, including those with various disabilities.
  • Ability to collaborate effectively within SFC and with other community organizations.
  • Strong organizational and time management skills.
  • Reliable transportation, valid Colorado Driver's License, and good driving record required.
