Job Title: Senior Case Manager

Samaritan Daytop Village, Inc. is seeking a highly skilled and experienced Senior Case Manager to join our team.

About the Role:

The Senior Case Manager will be responsible for providing supervision and guidance to Case Managers, ensuring the delivery of high-quality services to clients. This role will involve coordinating and guiding the activities of direct reporting staff, providing direct supervision of Case Managers, and leading and participating in administrative and staff meetings.

Key Responsibilities:
  • Communicate all aspects of contract requirements to staff
  • Coordinate, guide, and supervise activities of direct reporting staff
  • Provide direct supervision of Case Managers
  • Lead and participate in administrative and staff meetings
  • Troubleshoot problems and make decisions in accordance with program policies, procedures, and protocols
  • Monitor clients' progress towards their goals and document via progress notes
  • Conduct periodic review of all case documentations to ensure accuracy, completeness, and clarity
  • Conduct chart auditing of client files
  • Directly manage a case portfolio of approximately 25 clients
  • Participate in monitoring therapeutic environment to ensure that behavioral guidelines are followed, a therapeutic milieu is maintained, and interpersonal relationships remain positive
  • Provide individual counseling and facilitation of caseload group meetings and other assigned groups
  • Provide psycho-education and counseling germane to chemical dependency and relapse prevention
  • Administer appropriate behavioral interventions as needed
  • Participate in assessment screenings to determine client needs/preferences; formulate and implement treatment, transition, and discharge/continuing care planning collaboratively with individual clients
  • Facilitate re-socialization process of clients by serving as a role model
  • Provide role modeling regarding ethical and professional conduct
  • Keep abreast of changes in the field
  • Perform other duties as requested
Requirements:

To be successful in this role, you will need:

  • H.S Diploma/GED with Five (5) years' experience preferably supervising Case Managers within transitional/shelter programs or related setting
  • OR Associates Degree in Human Services or related field with at least Two-Three (2-3) years' experience supervising Case Managers within transitional/shelter programs or related setting
  • OR Bachelor's Degree in Human Services with at least One-Two (1-2) years' experience supervising Case Managers within transitional/shelter programs or related setting
  • Knowledge of CARES a plus
  • Excellent time management
  • Experience working directly with people from diverse racial, ethnic, and socioeconomic backgrounds
  • Working knowledge of substance abuse treatment modalities and client self-help/support modalities
  • Strong writing skills, competencies in writing goal-based/person-centered treatment plans and progress notes
  • Computer literacy including proficiency in Microsoft Office Suite and EMR
Preferred Qualifications:
