Job Title: Case Manager

Overview

The W Group Assisted Living Company is seeking a compassionate and dedicated Case Manager to join our team. As a Case Manager, you will play a vital role in coordinating services for residents, ensuring their needs are met, and providing exceptional care.

Key Responsibilities:
  • Maintain a high level of resident satisfaction by adhering to their "Bill of Rights"
  • Establish and maintain effective communication with resident care providers
  • Assess and coordinate admission/re-admission from the community, nursing homes, and hospitals
  • Supervise clinical care for residents and ensure clear and concise documentation
Requirements:
  • Prior experience in Long Term Care or 2 years' experience as a Case Manager
  • Previous case management and Compuware experience a plus
  • Excellent phone skills and ability to work with families/other care providers
  • Strong organizational skills, passion, and compassion for the population
