Job Title: Senior Case Manager

Samaritan Daytop Village, Inc. is seeking a highly skilled and experienced Senior Case Manager to join our team.

About the Role

As a Senior Case Manager, you will be responsible for providing case management services to clients requiring social service assistance. You will work collaboratively with clients to interview and evaluate their needs, formulate Independent Living Plans, and locate and utilize community resources to support their goals.

Key Responsibilities
  • Communicate contract requirements to staff and coordinate activities of direct reporting staff.
  • Provide direct supervision of Case Managers and lead administrative and staff meetings as required.
  • Troubleshoot problems and make decisions in accordance with program policies, procedures, and protocols.
  • Monitor clients' progress towards their goals and document via progress notes.
  • Conduct periodic review of case documentations to ensure accuracy, completeness, and clarity.
  • Directly manage a case portfolio of approximately 25 clients and participate in monitoring therapeutic environment.
  • Provide individual counseling and facilitation of caseload group meetings and other assigned groups.
  • Administer appropriate behavioral interventions as needed.
  • Participate in assessment screenings to determine client needs/preferences and formulate treatment, transition, and discharge/continuing care planning.
  • Facilitate re-socialization process of clients by serving as a role model and providing role modeling regarding ethical and professional conduct.
Requirements

To be successful in this role, you will need:

  • A high school diploma/GED with five years' experience preferably supervising Case Managers within transitional/shelter programs or related settings.
  • Or an associate's degree in Human Services or a related field with at least two-three years' experience supervising Case Managers within transitional/shelter programs or related settings.
  • Or a bachelor's degree in Human Services with at least one-two years' experience supervising Case Managers within transitional/shelter programs or related settings.
  • Knowledge of CARES is a plus.
  • Excellent time management and experience working directly with people from diverse racial, ethnic, and socioeconomic backgrounds.
  • Working knowledge of substance abuse treatment modalities and client self-help/support modalities.
  • Strong writing skills and competencies in writing goal-based/person-centered treatment plans and progress notes.
  • Computer literacy including proficiency in Microsoft Office Suite and EMR.
Preferred Qualifications

We are looking for candidates who can participate in CRP-related trainings and are bi-lingual in Spanish.
