Community Health Outreach Specialist

The Community Health Outreach Specialist supports the operational and administrative functions of a multi-year program designed to reduce health disparities in underserved communities.

This is a grant-funded position that requires strong community outreach and engagement skills.

Responsibilities:

  • Conduct door-to-door surveys and assessments in assigned areas to identify community health needs.
  • Support planning and coordination of program activities and operations.
  • Coordinate and maintain applicable program schedules and agendas.
  • Help deliver general welfare interventions to assigned communities.
  • Provide services linkages to community members.
  • Attend community outreach events.
  • Use software apps and tools to collect and report relevant data tracking community health metrics.

Scope and Travel:

Extensive travel by foot and by car to, from, and within assigned areas, as well as travel to and from CORE's Los Angeles Headquarters.

Qualifications:

  • Preference for candidate with bachelor's degree in public health or health sciences, or at least two years' equivalent experience in a relevant field.

Experience:

  • Minimum 1 year experience in public health, healthcare settings, community outreach, or related fields.
  • Experience in community outreach, conducting surveys, collecting data, and reporting data.
  • Experience working with community-based organizations, local government agencies, volunteer groups in LA County (preferred).

Skills and Competencies:

  • Ability to work independently and as part of a team.
  • Excellent written and verbal communication skills.
  • Proficient in MS Office applications.
  • Flexible and adaptive to a frequently changing environment.
  • Excellent organizational and problem-solving skills.
  • Close attention to detail and a high level of accuracy.
  • Ability to walk 4-5 miles/day (reliable transportation required).
  • Knowledgeable of HIPAA regulations.

REQUIRED:

  • Reading, writing, and speaking fluency in English, as well as one of the following languages: Spanish, Chinese (Mandarin and/or Cantonese), and/or Vietnamese.
