  • Provide outreach and engagement services to veterans in need, including case conferencing and coordination with local providers.
  • Work closely with the Los Angeles Homeless Service Authority (LAHSA) to manage the Veteran's By-Name List and coordinate with housing navigation teams.
  • Administer the Vulnerability Index-Service Prioritization Decision Assistance Tool (VI-SPDAT) to assess the needs of homeless contacts established through outreach efforts.
  • Maintain collaborative relationships with partner agencies, including the Veterans Administration and other community partners.
  • Attend staff trainings and department meetings as needed, and perform other duties as assigned.
Requirements
  • Two years of experience working in a related position that involved the coordinated entry process, assessment, reporting, and case conferencing.
  • Prior experience working with individuals living with mental illnesses and a history of chronic homelessness.
  • Ability to coordinate with multiple agencies and service providers to facilitate assessment for individuals experiencing homelessness.
  • Proficient with Service Prioritization Decision Assistance Tool (SPDAT) assessments and triage tools.
  • Possess excellent organization skills, initiative, and follow-through.
  • Be detail-oriented, able to set priorities, meet deadlines, maintain a professional attitude, and conduct and appearance at all times.
  • Proficient in office software, including Word, Excel, Outlook, HMIS, and Internet use.
Education and Other Requirements
  • Bachelor's degree preferred, work experience can substitute.
  • Valid Driver's License, reliable transportation, current vehicle registration, proof of insurance, and the ability to be insured by New Directions.
  • Ability to successfully complete a background check, drug test, and Annual TB test.
