Case Manager, E6

The People Concern is seeking a highly motivated and compassionate Case Manager, E6 - Outreach Specialist to join our team. As a key member of our outreach team, you will be responsible for identifying, engaging, and providing support to homeless individuals in the Midtown Los Angeles area. You will work collaboratively with other team members to remove barriers to permanent housing and provide case management and aftercare services to clients placed in permanent housing.
Key Responsibilities:
- Identify and engage homeless individuals in the Midtown Los Angeles area
- Provide case management and aftercare services to clients placed in permanent housing
- Work collaboratively with other team members to remove barriers to permanent housing
- Develop effective, trusting relationships with clients that facilitate provision of services
- Maintain and record accurate electronic records, client files, and statistical information
Requirements:
- BA degree preferred or equivalent experience
- Demonstrated knowledge of and experience with the issues faced by the population served (homelessness, mental illness, substance abuse, medical issues, and domestic violence)
- Excellent human relations/communication skills; ability to act in professional manner regardless of circumstances
- Skilled in non-violent crisis intervention
- Ability to self-motivate and organize time effectively and efficiently
