Hospice Care Specialist

We are seeking a compassionate and dedicated Hospice Care Specialist to join our team at Medi Home Health & Hospice in Pulaski, VA.
The successful candidate will provide support services to patients in their homes, focusing on enhancing their physical and emotional comfort. This role requires strong communication skills, empathy, and the ability to work collaboratively with healthcare professionals.
Responsibilities- Provide personal care and activities of daily living assistance as needed, following the patient's plan of care.
- Apply safety principles and proper body mechanics when performing tasks such as ambulation, transferring patients, and assisting with range of motion and positioning.
- Prepare and serve adequate nutrition and fluid intake, ensuring that medically prescribed diets are followed under supervision.
- Observe and report changes in patient condition or expectations of service promptly to a skilled nurse via telephone and document communication in clinical notes.
- High school diploma or General Education Degree (GED) required.
- Documented completion of a state-recognized Certified Nursing Assistant course.
- Valid Certified Nursing Assistant certification as required by the state.
- Valid driver's license and company-required auto-liability insurance.
