Hospice Care Professional

We are seeking a compassionate and skilled Registered Nurse to provide high-quality care to our hospice patients in Rock Hill, SC.
As a member of our multidisciplinary team, you will work under the general direction of our Director of Professional Services or Hospice Administrator to support patients and families in maximizing their comfort and health.
- Develop and implement personalized care plans for patients.
- Maintain accurate records and communicate effectively with physicians and colleagues.
- Collaborate with the patient's healthcare team to ensure comprehensive care.
Responsibilities also include assisting with providing supportive care in accordance with attending physician orders and participating in on-call rotations as needed.
To be successful in this role, you will have:
- A successful completion of an accredited registered nursing program.
- Current licensure as a registered nurse in South Carolina.
- A minimum of one year experience in direct patient care.
- Prior experience in hospice nursing is strongly preferred.
