Job Description

Overview

The Arkansas Hospice, Inc. is seeking a highly skilled Registered Nurse Case Manager to oversee the care of our hospice patients. As a key member of our interdisciplinary team, you will play a vital role in ensuring our patients receive the highest quality care and support throughout their journey.

Key Responsibilities

  • Manage patient care functions after initial admission and assignment to a designated RN Case Manager.
  • Assist in the admission process as requested.
  • Interpret, when necessary, Arkansas Hospice, Inc. policies and procedures for nursing staff and others to assure understanding and compliance.
  • Coordinate and collaborate with interdisciplinary team members for the management of the plan of care on an ongoing basis to meet patient/family needs.
  • Update the Plan of Care on an ongoing basis, keeping other team members current regarding the progress of the patient's physical status and any changing needs for care.

Requirements

  • Knowledge of palliative care with emphasis on symptom control associated with terminal illness.
  • Knowledge of the holistic approach toward patient care and the nursing process as it applies to hospice patients and families.
  • Knowledge and acceptance of hospice philosophy and principles of care.
  • Ability to recognize both positive and negative aspects of death and grief.
  • Ability to communicate effectively both orally and in writing.
  • Ability to work as a team player.
  • Ability to demonstrate tact, resourcefulness, patience and dedication.
