Job Overview
The Personal Care Attendant is responsible for addressing the daily living and safety requirements of assigned participants, while assisting licensed nursing professionals in delivering care through various participant support activities and related non-professional services.

Key Responsibilities
  • Attend to participants' personal hygiene and essential needs promptly and with compassion within the PACE Center or community environment.
  • Support participants in daily living activities, including feeding, mobility, and other essential tasks.
  • Respond to participant requests efficiently or seek appropriate assistance when necessary.
  • Assist with assigned non-sterile procedures and other designated tasks to support participant care.
  • Ensure participant comfort, availability of necessary supplies, and maintenance of physical privacy.
  • Gather data and document participant care activities accurately on designated forms.
  • Maintain the accuracy and timeliness of recorded information in accordance with established protocols.
  • Adhere to established procedures consistently.
  • Respond promptly to call lights and fulfill participant requests or obtain necessary assistance.
  • Maintain clinical and age-specific competencies as defined for the assigned unit.
  • Provide participants with safety-related instructions aligned with their care plans and recovery, demonstrating appropriate health and safety practices.
  • Ensure cleanliness and organization of the participant's immediate surroundings to enhance comfort.
  • Contribute to maintaining an orderly and clean participant care unit by organizing supplies and equipment areas.
  • Actively participate in ensuring continuity of participant care.
  • Monitor participants for emotional, behavioral, and physical changes, including mood shifts, skin condition, bowel and bladder status, food/fluid intake, speech, mobility, and transfer capabilities.
  • Report any changes in participant condition to nursing staff promptly, following established procedures and communication channels.
  • Provide homemaking services, including meal preparation and serving according to nutritional needs and dietary restrictions outlined in the care plan.
  • Follow procedures and policies diligently, using equipment and supplies efficiently while alerting management to any issues or needs.
  • Perform light housekeeping tasks as specified in the care plan, including cleaning and organizing.
  • Other responsibilities as assigned.
Qualifications & Training
  • Relevant experience in participant care, including skills such as temperature, pulse, respiration monitoring, blood pressure measurement, and assistance with activities of daily living.
  • High School diploma or GED, with the ability to read, write, and comprehend verbal or written instructions.
  • Minimum of one year of personal or professional experience providing care or services to frail or elderly individuals.
  • Completion of food handling training and certification upon hire or within the first month of employment.
  • CPR certification required upon hire.
Skills & Competencies
  • Preferred knowledge of the PACE program.
  • Ability to work independently without close supervision and exercise sound judgment.
  • Understanding of the physical, mental, and social needs of frail older adults.
  • Able to adapt to changing priorities based on participant needs.
  • Demonstrates necessary skills and knowledge as outlined in the position-specific Competency Assessment Profile.

  • Occasional travel between various sites, including nursing/group homes and participants' residences may be required.
  • Use of a personal vehicle is necessary.
  • A valid driver's license is required.
  • Proof of automobile insurance coverage at specified minimum amounts is necessary for mileage reimbursement.
  • May require the use of a personal cell phone for business purposes, with potential eligibility for a stipend.
