Position Overview

Key Responsibilities

  • Collaborate with a multidisciplinary team to formulate, implement, monitor, and refine comprehensive care plans that align with shared objectives and measurable outcomes.
  • Conduct thorough assessments of patient health through interviews, examinations, and review of medical histories.
  • Diagnose medical conditions, order and interpret laboratory tests and diagnostic studies in accordance with established protocols.
  • Maintain accurate documentation of all patient interactions and ensure medical charts are signed within the required timeframe.
  • Develop treatment strategies that address both dysfunction and disease while minimizing the risk of further injury.
  • Prescribe medications as permitted by state licensure and institutional privileges.
  • Utilize evidence-based methods to facilitate shared decision-making that respects patient preferences and cultural considerations.
  • Order necessary therapies, including occupational and physical therapy, and perform injections as per training and privileges.
  • Empower patients to take charge of their health through self-management and community resource utilization.
  • Engage with patients in both individual and group settings to achieve care plan objectives and provide education.
  • Provide care for a medically complex and emotionally challenging patient demographic, demonstrating compassion and the ability to de-escalate difficult situations.
  • Adhere to all federal, state, and local legal and professional standards by staying informed about current and upcoming legislation and advising management on necessary actions.

Qualifications

  • Current licensure as a Physician Assistant or Nurse Practitioner in California.
  • Preferred: 1+ years of relevant experience in a similar role.
  • Preferred: Bilingual in English and Spanish.

Work Environment and Physical Demands

The role requires the ability to sit, manipulate equipment, communicate effectively, and perform physical tasks such as standing, walking, and lifting up to 75 pounds. Reasonable accommodations may be made for individuals with disabilities.
