Key Responsibilities
  • Conduct spiritual assessments of patients and families to identify their spiritual needs and preferences.
  • Develop and implement individualized spiritual care plans to meet the unique needs of each patient and family.
  • Provide spiritual support and guidance to patients and families, including prayer, counseling, and education.
  • Collaborate with our interdisciplinary team to ensure that spiritual care is integrated into the overall care plan.
  • Document spiritual care provided and maintain accurate records.
  • Participate in patient care conferences and interdisciplinary team meetings to ensure that spiritual care is coordinated with other aspects of care.
  • Recruit and coordinate with community clergy and spiritual counselors to provide additional spiritual support to patients and families.
  • Develop and maintain relationships with community clergy and spiritual counselors to ensure that spiritual care is accessible and available to patients and families.
  • Participate in quality improvement initiatives to ensure that spiritual care is provided in a timely and effective manner.
Requirements
  • Master's degree in divinity or a related field.
  • Experience working in hospice care or a related field.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Ability to maintain confidentiality and handle sensitive information.
  • Valid driver's license and reliable transportation.
Working Conditions

This position requires frequent travel to patient homes and healthcare facilities. The ability to work in a fast-paced environment and adapt to changing circumstances is essential. The successful candidate will be able to work independently and as part of a team, and will be able to maintain confidentiality and handle sensitive information.
