Job Overview
The Personal Care Attendant is responsible for addressing the daily living and safety requirements of assigned participants, while assisting licensed nursing professionals in delivering care through various participant care activities and related non-professional services.

Key Responsibilities
  • Attend to participants' personal hygiene and essential needs promptly and compassionately within the PACE Center or community.
  • Assist participants with daily living activities, including feeding, mobility, and other essential tasks.
  • Respond to participant requests efficiently or seek appropriate assistance when necessary.
  • Perform and assist with designated non-sterile procedures and other tasks that support participant care.
  • Ensure participant comfort, availability of necessary supplies, and maintenance of physical privacy.
  • Document and record participant care activities accurately on the appropriate forms.
  • Maintain accuracy and timeliness in information recording, adhering to established guidelines.
  • Follow established protocols and procedures diligently.
  • Respond promptly to call lights and fulfill participant requests or seek appropriate assistance.
  • Maintain clinical and age-specific competencies as defined for the assigned unit.
  • Provide safety-related instruction to participants in line with their care plans and recovery, demonstrating appropriate health and safety practices.
  • Keep the immediate participant environment clean and organized to ensure comfort.
  • Assist in maintaining an orderly and clean care unit by organizing supplies, linens, and equipment areas.
  • Actively participate to ensure continuity of participant care.
  • Observe participants for emotional, behavioral, and physical changes, including mood shifts, skin condition, and mobility status.
  • Report any changes in participant condition to nursing staff promptly, following proper procedures and communication channels.
  • Provide homemaking services, including meal preparation and serving, according to nutritional needs and dietary restrictions outlined in the care plan.
  • Adhere to procedures and policies, using equipment and supplies efficiently, and alert management to any issues regarding policies or practices.
  • Perform light housekeeping tasks as specified in the care plan, including cleaning and laundry duties.
  • Other duties as assigned.
Education & Training
  • Relevant experience in participant care, including skills such as: 1) temperature, pulse, respiration; 2) blood pressure; 3) bed bathing; and 4) activities of daily living.
  • High School diploma or GED, with the ability to read, write, and comprehend verbal or written instructions.
  • Minimum of 1 year of personal or professional experience providing care or services to a frail or elderly population.
  • Completion of food handling training and certification upon hire or within 30 days of employment.
  • CPR certification required upon hire.
Skills & Abilities
  • Preferred knowledge of the PACE program.
  • Ability to work independently without close supervision and exercise sound judgment.
  • Understanding of the physical, mental, and social needs of frail older adults.
  • Able to adapt to changing priorities based on participant needs.
  • Demonstrates the necessary skills and knowledge as outlined in the position-specific Competency Assessment Profile.

  • Must be able to adhere to the required schedule.
  • Physical strength is necessary to perform essential job functions.
  • Occasional travel between various sites, including nursing/group homes and participants' residences, may be required.
  • Use of a personal vehicle is necessary.
  • A valid driver's license is required.
  • Proof of automobile insurance coverage at specified minimum amounts is necessary for mileage reimbursement.
  • May require the use of a personal cell phone for business purposes (eligibility for stipend may apply).
