Interim Healthcare Hospice is seeking a compassionate and dedicated Per Diem Chaplain to provide spiritual care to patients and their families in Victorville, CA.

Role Overview:

The Per Diem Chaplain will be responsible for assessing and addressing the spiritual needs of hospice patients and their families. This includes providing direct spiritual services, coordinating with community clergy, and participating in care planning.

Key Responsibilities:

  • Conduct spiritual assessments of patients and families to understand their unique needs and preferences.
  • Coordinate and provide spiritual services in a timely and compassionate manner, ensuring that patients and families receive the support they need.
  • Offer direct spiritual care based on individual beliefs and values, respecting the diversity of patients and families.
  • Participate in care planning meetings to ensure that spiritual care is integrated into the overall care plan.
  • Conduct religious services as needed, providing a sense of community and connection for patients and families.
  • Document services provided and maintain accurate records to ensure continuity of care.
  • Serve as a liaison to community clergy, fostering collaboration and support for patients and families.
  • Arrange funeral or memorial services, providing a dignified and respectful farewell for patients and families.
  • Provide support and education on spiritual care, empowering patients and families to make informed decisions about their care.

Requirements:

  • Graduate of an accredited seminary or theological school, with a strong foundation in spiritual care and counseling.
  • Experience working with end-of-life care, with a deep understanding of the unique challenges and needs of patients and families.
  • Knowledge of hospice philosophy and principles, with a commitment to providing compassionate and person-centered care.
  • Strong communication and organizational skills, with the ability to work effectively in a team and prioritize multiple tasks and responsibilities.
  • Ability to work effectively in a fast-paced environment, with a high level of adaptability and flexibility.
  • Valid driver's license and auto insurance, with the ability to travel between locations as needed.
  • Ability to meet health and background check requirements, ensuring a safe and secure work environment.

Working Conditions:

  • Work primarily in patients' homes or care facilities, with occasional travel to other locations as needed.
  • Possible exposure to infectious materials, with a commitment to following safety protocols and guidelines.
  • Ability to stand, walk, and perform physical tasks, with a focus on maintaining a safe and healthy work environment.
  • Flexible schedule may be required, with a commitment to providing 24/7 coverage as needed.
