Your duties will include:

  • Conducting in-home assessments and coordinating medical care for seniors.
  • Acting as a liaison between healthcare providers and families.
  • Providing education and counseling to families.
  • Documenting services provided accurately and promptly.
  • Participating in an on-call rotation as part of a team.

Qualifications:

  • Must be a licensed LPN, RN, or Social Worker with experience in elderly care or case management.
  • Availability for weekday and occasional evening/weekend on-call duties.
  • Home office setup required, including a scanner/printer and smartphone.
  • A valid driver's license and proof of insurance are necessary.
  • Two professional references from previous employers are required.
