Key Responsibilities
- Facilitate the RAI Process under the guidance of an R.N. or DNS.
- Collaborate with the interdisciplinary team to assess patient/resident needs.
- Coordinate and lead daily Case Management meetings.
- Ensure timely completion of MDS assessments in compliance with state and federal regulations.
- Oversee that the interdisciplinary team accurately completes designated sections of the MDS assessment.
- Conduct thorough coding of MDS assessments based on medical record reviews, observations, and interviews.
- Maintain a tracking system for MDS assessment schedules, including timeframes and due dates.
- Facilitate Care Plan conferences with the interdisciplinary team, patients, residents, and families.
- Review and maintain all State and Federal reports, ensuring timely corrections as needed.
- Monitor Quality Measures to ensure MDS accuracy aligns with these metrics.
- Provide education related to the RAI Process.
- Coordinate electronic submissions of required documentation to the State database and other entities as per company guidelines.
- Ensure compliance with Medicare and Medicaid regulatory guidelines.
- Engage in Managed Care case management at the center level.
- Review Additional Documentation Requests (ADRs) with the Health Information Management Coordinator.
- Participate in billing reviews to ensure claims are supported by MDS assessment documentation.
- Engage in Quality Assurance and Process Improvement initiatives.
- Stay updated on knowledge related to Medicare, Managed Care, Medicaid, RAI Process, and technological advancements.
- May be required to perform direct care nursing functions as needed.
Qualifications
- Current nursing license in the state of employment.
- Experience in Long-Term Care settings.
- Proficient computer skills.
- Knowledge of Medicare, Managed Care, and RAI Process is essential.
- Participation in the LPN to RN Back to School transition program is preferred.
