About the Role

Solutions Community Counseling and Recovery Centers is seeking a compassionate and dedicated individual to join our team as a Mental Health Case Manager. In this role, you will provide supportive services to individuals with mental illness, teaching daily living skills and ensuring safety and well-being for our clients.

Key Responsibilities
  • Provide emotional support to clients, ensuring progress by monitoring emotional and mental health, treatment plans, and other items.
  • Assist clients and/or family members by securing services and resources, including housing, medical treatment, financial assistance, and transportation.
  • Collaborate with clients and/or family members or client representatives by discussing or responding to treatment plans and other needs.
  • Advocate for clients in the community and with other agencies, including Human Services, Social Security Administration, and County and Common Pleas Courts.
  • Communicate with agency service providers and other professionals by attending meetings and training sessions.
Requirements
  • Bachelor's degree in human services or a related field, or a QMHS+3 years of experience.
  • Minimum of 3 months of experience working in a similar environment.
  • The ability to respond promptly to emergency or crisis situations.
  • Understanding and compassion for both clients and family members.
  • Strong organizational skills and the ability to follow up in a timely manner.
  • The ability to work independently as well as in a team environment.
  • The ability to communicate effectively both verbally and in writing.
  • Adaptable to teaching various learning styles.
