The Blended Case Management program at Hall-Mercer offers specialized case management services to adults in Philadelphia who are recovering from significant mental health challenges, potentially coupled with substance use disorders and/or homelessness. Our approach utilizes a team model to connect clients with essential support services, fostering stability and community integration.

Key Responsibilities:

  • Deliver targeted case management services to adults in Philadelphia dealing with severe mental health issues, including potential co-occurring substance use disorders and homelessness. The Case Manager aids clients in accessing services, monitoring service delivery, conducting assessments, and developing service plans. This role emphasizes problem-solving, building supportive networks, and leveraging community resources to empower clients to achieve their highest potential.

Essential Duties:

  • Ensure compliance with service delivery standards as mandated by relevant regulatory bodies. Maintain regular contact with clients, ensuring at least one billable service every two weeks and face-to-face meetings in clients' homes as required.
  • Document all services provided accurately and in a timely manner, adhering to the clinical documentation policies set forth by Hall-Mercer and relevant authorities.
  • Participate actively in team meetings and collaborate effectively with team members and supervisors regarding service outcomes.
  • Maintain productivity standards by providing an average of 16 billable service units daily.
  • Complete all necessary training and professional development activities, ensuring compliance with annual educational requirements.

Qualifications:

  • Basic Cardiac Life Support certification.
  • High School Diploma or GED (Required) with a minimum of 4 years of experience in an academic medical setting.
  • Bachelor's degree in a relevant field (Required) with at least 2 years of experience in health information management or medical coding.
