About the Role:

WES is seeking a dedicated and experienced professional to fill the position of Case Manager. The ideal candidate will be responsible for coordinating and managing a diverse caseload of clients, ensuring they receive the necessary resources and support to achieve their goals.

Key Responsibilities:

  • Manage a diverse caseload of clients, providing individualized support and guidance to help them navigate complex social service systems.
  • Conduct comprehensive assessments to determine client needs and develop tailored care plans that address their unique circumstances.
  • Coordinate services and resources for clients, including referrals to community organizations and social service agencies.
  • Monitor client progress and adjust care plans as necessary to ensure successful outcomes and optimal client satisfaction.
  • Maintain accurate and up-to-date documentation of all client interactions and services provided, ensuring compliance with regulatory requirements and company policies.
  • Communicate effectively with clients, families, and other stakeholders to foster collaboration and ensure seamless service delivery.
  • Provide crisis intervention and support as needed, ensuring client safety and well-being.
  • Participate in regular team meetings and case reviews to discuss client progress and share insights, promoting a culture of continuous improvement and excellence.

Requirements:

  • Bachelor's degree in Social Work, Psychology, or a related field required.
  • Minimum of 3 years of experience in case management or a related field, with a proven track record of success in coordinating services and supporting clients.
  • Strong understanding of community resources and social service systems, with the ability to navigate complex systems and identify opportunities for clients.
  • Excellent communication and interpersonal skills, with the ability to build rapport with clients and stakeholders from diverse backgrounds.
  • Ability to work independently and manage a high caseload effectively, with strong organizational skills and attention to detail.
  • Experience with crisis intervention and conflict resolution, with a focus on de-escalation and client-centered approaches.
  • Familiarity with case management software and documentation practices, with the ability to adapt to new systems and technologies.
