Responsibilities

As a Case Management Specialist, you will oversee the quality and resource management of all patients admitted to our facility, ensuring excellence from the point of admission through the entire continuum of healthcare management.

Your role will involve advocating for patients, promoting cost-effective care, and demonstrating leadership to seamlessly integrate healthcare providers for optimal patient outcomes.

This position is designed to facilitate and ensure the achievement of quality, clinical, and cost-effective results, performing comprehensive admission and concurrent reviews of medical records to assess medical necessity, service intensity, and illness severity.

Qualifications

EDUCATION, EXPERIENCE, TRAINING

  • A minimum of 5 years of professional experience post-graduation from an accredited nursing program and a current state Registered Nurse license.
  • For those grandfathered prior to specific criteria, a minimum of 5 years post-graduation from an accredited School of Social Work for Licensed Clinical Social Workers is acceptable, although RN Case Manager is preferred.
  • Five years of acute care nursing experience is preferred, with at least one year in case management, discharge planning, or nursing management.
  • Current BCLS (AHA) certification upon hire and maintenance of certification is required.
  • Familiarity with Milliman Criteria and InterQual Criteria is preferred.
  • Basic to intermediate computer skills are necessary.
  • Behavioral Violence Prevention Training must be completed within three months of hire and maintained thereafter.
