Job Summary:
The Case Manager Specialist will be responsible for providing blended case management services to adults 18 and over with a serious mental health diagnosis. You will work closely with our clients to assess their needs, develop personalized service plans, and link them to community resources to promote stability and independence.
Key Responsibilities:
- Assess clients' needs and develop service plans in conjunction with the client, family members, and other relevant stakeholders.
- Link clients to community resources, including mental health services, housing, employment, and education.
- Monitor clients' progress, adjust service plans as needed, and provide ongoing support to ensure successful outcomes.
- Collaborate with healthcare providers, social services, and other community agencies to ensure comprehensive care.
- Document client interactions, service plans, and progress in a timely and accurate manner.
- Participate in daily team meetings, individual supervision, and training to stay up-to-date on best practices and industry developments.
Requirements:
- Bachelor's degree in Social Work, Psychology, or a related field.
- At least 2 years of experience in case management, social work, or a related field.
- Fluency in English and Spanish preferred.
- Valid driver's license required.
- Ability to work in a fast-paced environment, prioritize tasks, and manage multiple cases simultaneously.
