Job Overview

Position Title: Medical Case Manager

Reports to: Case Management Coordinator

Department: Direct Services

Program: Case Management / Client Services

Status: Full Time, Non-Exempt, Eligible for benefits

Qualifications:

  • Bachelor's degree in social work, psychology, or a related field is required; Addiction Counseling Certification is a plus. A Master's degree is preferred.
  • Proven experience in case management, supportive housing, and client advocacy.
  • Ability to engage with diverse populations, particularly in critical and emergency situations.
  • Awareness of cultural, economic, gender, and sexual orientation differences, with a compassionate approach to individuals with disabilities.
  • Familiarity with local resources and service systems, along with an understanding of medical and psychosocial issues related to HIV, mental health, and substance abuse.
  • Willingness to work flexible hours, including evenings and weekends, and possess a valid driver's license.

Core Responsibilities:

  • Deliver comprehensive HIV case management services to a caseload of 20 clients with intensive needs.
  • Collaborate with the Direct Services team to ensure high-quality service delivery.
  • Support formerly homeless individuals living with HIV/AIDS, psychiatric disabilities, and/or substance use disorders.

Key Duties:

  • Conduct intake assessments in line with established procedures.
  • Participate in an on-call rotation.
  • Perform face-to-face assessments to gather medical, demographic, and consent information from clients.
  • Develop service care plans in collaboration with clients and support team members, ensuring confidentiality.
  • Provide case management and advocacy for clients with HIV, substance abuse, and mental health histories.
  • Conduct bi-monthly home visits and accompany clients to medical appointments as necessary.
  • Assist clients in navigating social services and connecting with community resources.
  • Offer information and referrals, including follow-up and monitoring of related activities.
  • Educate clients' support systems as needed, with informed consent.
  • Deliver tailored services to meet individual client needs, including housing placement and vocational guidance.
  • Consult with care providers on discharge planning and home care strategies.
  • Engage with various stakeholders to coordinate services for clients.
  • Participate in activities outside of standard business hours as required.
  • Provide intake coverage as assigned.

Performance Expectations:

  • Maintain knowledge of resources and entitlements to effectively advocate for client needs.
  • Complete necessary reports as determined by job function.
  • Provide leadership and support to team members as needed.
  • Act as a liaison with external collaborators to facilitate service coordination.
  • Adhere to clinical and procedural policies as outlined in the Direct Services Manual.
  • Attend relevant meetings and training sessions to stay informed and engaged.
  • Document client interactions and maintain accurate records as required.
  • Ensure compliance with contact frequency guidelines for assigned clients.
