About the Role

Discovery At Home is seeking a skilled Occupational Therapy Assistant to join our team in providing high-quality care to our patients in their homes. As an Occupational Therapy Assistant, you will work under the supervision of an Occupational Therapist to provide skilled occupational therapy services to clients in their homes.

Key Responsibilities
  • Client Care Services: Provide client care services under the direction of the Occupational Therapist, including evaluation and development of the rehabilitative plan of care and periodic re-evaluation as required.
  • Assessment and Documentation: Participate in the preparation of clinical and daily progress notes, and submit them within 72 hours for review by the Occupational Therapist. Prepare periodic summary reports and OASIS assessments, reassessments, transfers, resumptions of care, discharges, visit notes, and significant changes in condition in accordance with agency-defined time frames.
  • Education and Training: Instruct clients and family members on exercise programs, self-care activities, and the use of self-help devices. Guide and instruct clients in prescribed therapeutic activities to improve independent functioning and physical and mental well-being.
  • Collaboration and Communication: Consult with the Occupational Therapist regarding the outcome of home visits and report physical findings in written form. Participate in rehabilitation department staff meetings and case conferences, and attend patient care conferences as scheduled.
  • Quality and Compliance: Understand and adhere to established policies and procedures of the agency, and participate in Quality Assessment and Performance Improvement activities as assigned.
Requirements
  • Education: Graduate of an approved school giving a course of not less than two years for Occupational Therapy Assistants.
  • Licensure and Certification: Licensed or registered by the State, and CPR certification.
  • Criminal Background Check: Must have a criminal background check.
