Overview:
The Patient Care Assistant (PCA) is responsible for delivering essential patient care services and supportive activities under the supervision of a Registered Nurse or Licensed Vocational/Practical Nurse. This role focuses on providing quality care while adhering to the established policies and procedures of Post Acute Medical.

Key Responsibilities:
  • Collaborate with licensed staff to organize patient care and unit activities, ensuring timely completion of tasks and responsiveness to patient needs.
  • Maintain a clean and orderly environment in patient rooms to facilitate effective nursing care.
  • Deliver designated patient care under the direct supervision of a licensed nurse, following physician orders and established care standards.
  • Monitor and document vital signs, reporting any significant changes to the supervising nurse.
  • Record intake and output for assigned patients, alerting nursing staff to any unusual findings.
  • Document care provided and patient responses accurately and comprehensively.
  • Assist with meal distribution and collection, ensuring patients receive proper nourishment.
  • Support patients with daily living activities, including personal hygiene, mobility, and elimination, while promptly reporting any changes in their condition.
  • Uphold patient rights and dignity in all care practices, including appropriate use of restraints and safety measures.
  • Implement infection control protocols in all patient care activities.
  • Adapt care practices to meet the specific needs of patients across different age groups.
  • Engage in personal development activities to enhance skills and knowledge relevant to patient care.

Qualifications:
Education:
High school diploma or equivalent preferred.
Certification as a nursing assistant (CNA) as required by state regulations. Current BLS certification is mandatory.

Experience:
Preferred experience of two (2) to three (3) years in an acute care, rehabilitation, or skilled nursing environment.

Skills and Abilities:
  • Effective verbal communication skills for interacting with patients, families, and staff.
  • Proficient in reading and writing English, including basic medical terminology.
  • Ability to document care accurately as directed by licensed personnel.
  • Strong problem-solving and judgment skills.
  • Organizational skills to manage nursing tasks efficiently.
  • Ability to build rapport and trust with patients and their families.
  • Flexibility to adapt to changing work conditions and patient needs.
