Discovery At Home is seeking a skilled Occupational Therapy Assistant to join our team in providing high-quality care to our patients in their homes. As an Occupational Therapy Assistant, you will work under the supervision of an Occupational Therapist to provide skilled occupational therapy services to clients in their homes.
Key Responsibilities- Client Care Services: Provide client care services under the direction of the Occupational Therapist, including assisting with evaluation and development of the rehabilitative plan of care and periodic re-evaluation as required.
- Documentation: Prepare and submit clinical and daily progress notes, as well as periodic summary reports, for review and submission by the Occupational Therapist.
- Education and Training: Instruct clients and family members on exercise programs, self-care activities, and the use of self-help devices, under the supervision of the Occupational Therapist.
- Communication: Consult with the Occupational Therapist regarding the outcome of home visits and report physical findings in written form.
- Team Collaboration: Instruct other health team personnel, including Home Health Aides and family members, on certain phases of occupational therapy, as needed.
- Quality Assurance: Participate in Quality Assessment and Performance Improvement activities, as assigned.
- Administrative Tasks: Complete and submit OASIS assessments, reassessments, transfers, resumptions of care, discharges, visit notes, and significant changes in condition, in accordance with agency-defined time frames.
- Education: Graduate of an approved school giving a course of not less than two years for Occupational Therapy Assistants.
- Licensure: Licensed or registered by the State.
- Criminal Background Check: Must have a criminal background check.
- CPR Certification: Must have CPR certification.
