Position Overview:

At Stony Brook Medicine, our Practical Nursing Specialists play a vital role in our healthcare team, addressing health issues through established nursing practices and methodologies, including case identification, health education, and patient counseling.

These professionals deliver care that supports and restores health, while strictly following medical protocols.

Key Responsibilities:

- Deliver and oversee nursing care to residents within the nursing unit, adhering to the medical and nursing care framework, departmental policies, and under the guidance of a registered nurse.

- Closely monitor residents to assess their responses to treatments, medications, and any changes in their health condition.

- Administer treatments to residents in accordance with the medical and nursing care guidelines and departmental policies.

- Prepare and dispense medications to residents as per the medical care plan and in line with departmental regulations.

- Assist residents with daily living activities, providing direct care, monitoring their status, and supervising non-professional staff.

- Document daily observations, relevant data, and information in residents' progress notes, and assist with periodic documentation reflecting resident care and status.

- Promptly communicate any changes or incidents to the immediate supervisor and complete necessary documentation, including incident reports and shift reports. Ensure adherence to medication management policies, including the handling of controlled substances.

- Collaborate with nursing staff and other departments to share important information affecting resident care. Address and respond to concerns raised by residents and their families.

- Provide direct care that aligns with comprehensive assessments, resident requests, and care plan objectives.

- Work in conjunction with other healthcare professionals to ensure coordinated and timely care delivery.

- Engage in the Quality Assurance Program and participate in committees focused on enhancing resident care quality.

- Assist in orienting new employees to the nursing unit and in the clinical training of non-professional staff.

Qualifications:
- New York State LPN License

Preferred Experience:
- Minimum of one year in a healthcare setting working with adults or the elderly. Experience in long-term care is advantageous. Familiarity with electronic medical records is preferred. A resume and cover letter should accompany the online application.

Work Environment:

The role may involve exposure to infectious diseases, bodily fluids, and other challenging elements, including accidents and illnesses. Employees may encounter unpredictable situations requiring immediate response and may occasionally work irregular hours due to various demands.

Essential Position:

This role is classified as essential, meaning that during emergencies, employees in this position are required to remain on-site or report to work.

New employees will need to complete a pre-employment physical examination and obtain medical clearance, pass a background check, provide three written references, and submit copies of required licenses or certifications.
