Job Summary

This is a challenging and rewarding role for a Licensed Practical Nurse to join our team at Stony Brook University. As a critical member of our healthcare team, you will be responsible for providing high-quality patient care and support to our residents.

Key Responsibilities
  • Provide and ensure nursing care to residents on the nursing unit within the framework of the medical and nursing care plan.
  • Monitor residents closely to identify responses to treatments, medication, and/or changes in health status.
  • Prepare and dispense medication to residents according to the medical plan of care.
  • Directly assist with residents' activities of daily living, including direct care, observation of status, and supervision of non-professional staff.
  • Document daily in residents' progress notes all pertinent data, information, and observations.
  • Communicate immediately all changes and/or incidents to the immediate supervisor and complete required documentation.
  • Maintain policies and procedures relevant to medications, including management of controlled substances.
  • Communicate to supervisor, nursing staff, and other departments and disciplines all pertinent information and changes impacting resident care.
  • Listen and respond appropriately to concerns or issues expressed by residents and families.
  • Ensure and provide direct care to residents that is consistent with the comprehensive assessment, request of the resident, and the goals of the plan of care.
  • Work closely with other professional and disciplines to ensure timely and coordinated care.
  • Participate in the Quality Assurance Program of the Department and serve on committees pertinent to the quality of care of residents.
  • Participate in the orientation of new employees to the nursing unit and the clinical in-service of non-professional staff.
Working Conditions

Working conditions may involve exposure to infectious disease, blood, body fluids, and/or tissue as well as other unpleasant elements such as accidents, injuries, and illness.

May be exposed to and be required to handle varying unpredictable, emergency, and crisis situations. Occasionally subjected to irregular hours and pressure due to multiple calls and inquiries.

Required Qualifications
  • A license and current registration to practice as a practical nurse in New York State.
  • A limited permit to practice as a practical nurse in New York State.
  • An application on file for a limited permit to practice as a practical nurse in New York State.
Preferred Qualifications
  • One year of healthcare experience working with an adult or aging population in a clinical setting.
  • One year of long-term care experience.
  • Experience using EMR.
