Position Overview:

At Stony Brook Medicine, our Practical Nursing Specialists play a vital role in our healthcare team, addressing and managing responses to existing or potential health concerns through established nursing techniques and methods. The incumbents are dedicated to providing care that supports and restores health and well-being while following prescribed medical protocols.

Key Responsibilities:

  • Deliver and oversee nursing care to residents within the framework of the medical and nursing care plan, adhering to departmental policies and under the guidance of a registered nurse.
  • Closely monitor residents to assess their responses to treatments, medications, and any changes in health status.
  • Administer treatments to residents in accordance with the medical and nursing care plan, ensuring compliance with departmental policies.
  • Prepare and dispense medications to residents as per the medical care plan and departmental regulations.
  • Assist residents with daily living activities, providing direct care, observing their status, and supervising non-professional staff.
  • Document daily in residents' progress notes all relevant data, information, and observations, and assist with comprehensive documentation reflecting resident status and care.
  • Communicate any changes or incidents to the immediate supervisor promptly and complete necessary documentation.
  • Maintain adherence to policies and procedures related to medication management, including controlled substances.
  • Collaborate with supervisors, nursing staff, and other departments to share important information affecting resident care.
  • Address and respond appropriately to concerns raised by residents and their families.
  • Ensure that direct care provided aligns with comprehensive assessments, resident requests, and care plan objectives.
  • Work in close coordination with other professionals to guarantee timely and integrated care.
  • Engage in the Quality Assurance Program and participate in committees focused on enhancing resident care quality.
  • Assist in the orientation of new staff members and provide clinical training for non-professional personnel.

Qualifications:

Required: Valid NYS LPN License.

Preferred: A minimum of one year of healthcare experience with adult or aging populations in a clinical environment, and one year of experience in long-term care settings. Familiarity with Electronic Medical Records (EMR) is advantageous.

Working Environment:

Employees may encounter exposure to infectious diseases, bodily fluids, and other challenging elements such as accidents and illnesses. The role may involve managing unpredictable situations and irregular hours due to the nature of healthcare demands.
