About the Role:
CHOICE is looking for dedicated Licensed Practical Nurses to join our team. In this role, you will assess and address the health needs of our residents through effective nursing practices, including case finding, health education, and supportive counseling.

Key Responsibilities:
As a Licensed Practical Nurse, your duties will encompass a variety of essential tasks, including but not limited to:

  • Delivering comprehensive nursing care to residents, adhering to established medical and nursing care plans under the guidance of a registered nurse.
  • Closely monitoring residents to identify their responses to treatments, medications, and any changes in their health conditions.
  • Administering treatments as per the medical care plan and in compliance with departmental policies.
  • Preparing and dispensing medications in accordance with the prescribed medical plan.
  • Assisting residents with daily living activities, ensuring their comfort and safety while supervising non-professional staff.
  • Documenting all relevant information and observations in residents' progress notes and contributing to periodic evaluations of resident care.
  • Promptly communicating any changes or incidents to your supervisor and completing necessary documentation.
  • Upholding policies related to medication management, including the handling of controlled substances.
  • Engaging with residents and their families to address any concerns or issues they may have.
  • Collaborating with interdisciplinary teams to ensure coordinated and timely care for residents.
  • Participating in quality assurance initiatives and contributing to committees focused on enhancing resident care.
  • Assisting in the orientation of new staff members and providing clinical training to non-professional personnel.

Qualifications:
To be considered for this position, candidates must possess:

  • A valid NYS LPN License.
  • A minimum of one year of healthcare experience with an adult or aging population in a clinical environment.
  • At least one year of experience in long-term care settings.
  • Proficiency in using Electronic Medical Records (EMR).

Working Conditions:
This role may involve exposure to infectious diseases, bodily fluids, and other challenging elements such as accidents and illnesses. Candidates should be prepared to handle unpredictable emergencies and may occasionally work irregular hours due to the demands of the position.

Available Shifts:
2nd shift (3 PM - 11 PM)
3rd shift (11 PM - 7:30 AM)
