Position Overview

NYAP is seeking a Bilingual Case Manager I for our Transitional Foster Care (TFC) Program. This role involves working with unaccompanied minors who arrive in the United States seeking refuge. These children are temporarily placed in various programs across the country before being reunited with sponsors in local communities. Our collaboration with federal agencies ensures that we provide essential case management services to facilitate the reunification process.

Key Responsibilities

The Bilingual Case Manager I will be responsible for:

  • Completing and submitting timely assessments and service plans.
  • Maintaining comprehensive case files in compliance with ORR policies.
  • Conducting on-site admissions and initial intake interviews with youth.
  • Interviewing sponsors and family members to assess their ability to care for minors.
  • Acting as the point of contact for assigned federal specialists and case coordinators.
  • Leading multidisciplinary team meetings to discuss case management strategies.
  • Coordinating legal presentations and screenings for children in care.
  • Documenting all interactions and maintaining accurate progress notes.
  • Facilitating communication between minors and their families or sponsors.
  • Ensuring a safe environment for children and timely release from care.
  • Participating in continuous quality improvement activities.

Qualifications

  • Bilingual in English and Spanish, with excellent communication skills.
  • Master's degree in social work or equivalent experience in family or child welfare.
  • Proficient in using technology and various office equipment.
  • Ability to work flexible hours, including evenings and weekends.
  • Must be at least 21 years of age and possess a valid driver's license.
